Linked from this page are questions to send in your
child's Care Package which a staff member at the Social
Welfare Institute (SWI) where she resides may fill
out for you. If at all possible, you should go over
these questions with a translator shortly after you
receive your child. Many of the questions are health
related, and the information is needed immediately.
Upon your return home, you can have the answers more
carefully translated, as a permanent record.
The answers to these questions may be the only history
your child will ever have of her life before she became
part of your family. This information will also be
invaluable in assembling your child's Life Book.

Note: some agencies no longer permit Care Packages
to be sent to China. Also, there is no guarantee of
the package arriving, or of the contents being given
to your child. We still think it's a risk worth taking!

Questions 1-3
- Where and how was she found and on what date?
- Who found her?
- How do you know her age?
Questions 4-7
- Was a note or any other mementoes found with
her?
- Who chose her name and why? Does it have any
special meaning?
- Is there a nickname people call her?
- How was her health the first year?
Questions 8-12
- Any prior illnesses, accidents or injuries? If
so, what was the treatment?
- Does she have any allergies?
- Has she had any immunizations?
- What kind of food is she eating now? How much?
When?
- What are her sleeping habits - how much does
she sleep and when? Does she sleep on tummy or back?
Questions 13-17
- Does she have any likes/dislikes/sensitivites?
- What is her personality like? What makes her
happy?
- What makes her angry or upset? How do you calm
and soothe her when she cries?
- Are there things I should buy here for her to
take home to keep for her?
- Was there a particular person she was attached
to?
Questions 18-20
- Was there one person who took care of her at the
orphanage or many?
- Did she go home with someone at night or sleep
at the orphanage?
- Does she drink from a bottle or a cup?
